1. Please complete the registration form & email a softcopy to bolt@livewiremedia.org with your proof of payment.    
Print out your completed form and bring it along to camp, making sure that the parental consent part is signed.

Helper   (age 21 & above, by invitation)          RM200        1 August 2012

(The fees above include 3 nights accommodation & 11 meals from 21-24 August 2012 & does NOT include transportation to or back from camp venue. Note that the 1st 40 campers to submit their full registration details AND full payment will be guaranteed their spaces at Bolt 2012.)

3. Payment modes

    Deposit: You can deposit cheques or cash at any Public Bank branch or deposit kiosk (PBB Account #: 3173964000),
   then email a digital copy of the deposit slip to bolt@livewiremedia.org with your registration form. Please make cheques     payable to LIVEWIRE MEDIA SDN BHD.
   
    Transfer: You can transfer funds online or at any Public Bank ATM machine into the
    LIVEWIRE MEDIA SDN BHD (PBB Account #: 3173964000), then email a digital copy of the transfer slip to
   bolt@livewiremedia.org with your registration form.

4. Confirmation of registration will be sent upon receipt of both the registration form & proof of payment.
