1. Please complete the registration form & email a softcopy to bolt@livewiremedia.org with your proof of payment.    
Print out your completed form and bring it along to camp, making sure that the parental consent part is signed.

Helper   (age 18 & above, by invitation)          RM200        1 May 2010

(The fees above include 3 nights accommodation & 11 meals from 8-11 June 2010 & does NOT include transportation to or back from camp venue. Note that the 1st 75 campers to submit their full registration details AND full payment will be guaranteed their space at Bolt 2010.)

3. Payment modes

    Deposit: You can deposit cheques or cash at any Maybank branch or deposit kiosk (MBB Account #: 514271253491),
   then email a digital copy of the deposit slip to bolt@livewiremedia.org with your registration form. Please make cheques    payable to LIVEWIRE MEDIA SDN BHD.
   
    Transfer: You can transfer funds online or at any Maybank ATM machine into the
    LIVEWIRE MEDIA SDN BHD (Maybank Account #: 514271253491), then email a digital copy of the transfer slip to
   bolt@livewiremedia.org with your registration form.

4. Confirmation of registration will be sent upon receipt of both the registration form & proof of payment.
